Healthy  Change is a referral and support service to improve your heart health.  They offer ongoing support over the phone to help people lose weight, stop smoking, become more active and/or to cut down on drinking and refer you to free services such as weight loss classes, exercise classes and health improvement schemes.  There is Healthy Change

Young Carers Transitions Project

A new project has been set up for young Carers aged 15-19 around transitioning from adolescence to adulthood. The Young Carers Transitions Project has developed a life skills programmes for young carers aged 15-19 as they approach adulthood. Crisis Prevention Service Nottinghamshire County The Crisis Prevention Service enables the person you care for to be looked after in their home if you have an unforeseen or emergency situation.
