A new project has been set up for young Carers aged 15-19 around transitioning from adolescence to adulthood.
The Young Carers Transitions Project has developed a life skills programmes for young carers aged 15-19 as they approach adulthood.
We offer a programme of group sessions and one to one mentoring support to enable young carers to develop their skills and confidence to make decisions about their future and become more independent.
Sessions will include health and well-being, positive relationships, communication and decision making, keeping safe, education and training, employability skills, etc. We can also offer advice and signposting to other services and opportunities and support to access these opportunities.
We provide a safe and confidential space to discuss concerns and worries and help people to explore and work towards the things that young carers identify as important to them, setting realistic and achievable goals.
